At about 9:00 a.m. on December 27, 2021, an employee was feeding a scrap sheet into a grinder when their right hand was caught between a bar (which kept tension on the sheet) and the steel frame of the machine guarding. The employee suffered bruising to the hand and was hospitalized.
An employee went to the south side of a machine to grab an extruded plastic sheet and realized the roller assembly was in the up position instead of down.
