An employee secured her pallet jack on a trailer, then turned the handle to lock it. The jack moved and pinned her right leg against the side of the trailer. She suffered a broken tibia and fibula.

An employee was unloading a pallet jack from a flatbed trailer. When the tire of the pallet jack contacted the wood decking of the trailer, the jack shifted abruptly and caused the employee to fall off the side of the trailer, landing on the ground. The employee suffered a laceration to the back of his head, a concussion, and a fractured right elbow.

On January 24, 2024, an employee was using a rag to clean a drum that rotates bottles for labeling. The rag got caught on the drum and pulled their right index finger into the point of operation, resulting in a fingertip amputation.

The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was cleaning a depositor machine to setup for a new product run. His right index finger was caught by moving parts in the depositor machine and the fingertip was partially amputated without bone loss.

An employee was under a label machine, working to remove a jammed label. The machine partially amputated her left index finger at the first knuckle.

An employee was operating an electric pallet jack. While standing beside the equipment, the pallet jack lurched forward, pinning the employee's leg against racking. The employee was hospitalized with a fractured right ankle.
