An employee was exiting a storage container. When he stepped down onto his right foot, the foot twisted outward and was broken.

A maintenance employee was working on a barge in open water, replacing the belt on a deck winch. After replacing the belt and testing it, the employee was taking a picture of the model number on the belt when the machine started. His hand was pulled into the belt and pulley, resulting in a partial amputation to his index finger.

An employee sustained a fracture while installing a new cable on a crane.

An employee was installing bolts using a pneumatic torque wrench when the wrench contacted their left index finger, resulting in amputation.

An employee was standing alongside a slip, with a mooring line next to the cleat, attempting to catch/slow down a barge and tie it up. The mooring line grabbed the employee's right heel and pulled it into the cleat, causing compound fractures to the right tibia and fibula and severe ankle lacerations.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

A marine diver/construction worker was working underwater, using a hydraulic dredge to clear sediment away from the base of a dam. The dredge's suction nozzle began migrating toward him, and then pulled in his right hand and forearm. He sustained compartment syndrome in the hand and forearm.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew. As he was moving to alert the crew to an incoming overhead load, he stepped off the plywood and his left foot slipped through the rebar mat and landed on the rebar mat below. His left ankle was broken and dislocated.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
