Hanna Steel Corporation

An employee had picked up a piece of steel tubing at each end to throw it into a recycling bin. When the employee threw the tube, it caught on a fixed air pipe. The employee's right hand was caught between the end of the tube and the pipe, and the employee's index finger was broken.

Mimbela Contractors Inc

As part of a sidewalk demolition project, the injured employee was loading rocks and concrete by hand into the bucket of a skid steer. A rock thrown by another employee struck the injured employee's left hand, causing a partial amputation of the middle finger.

A temporary employee was picking up a metal bed frame and throwing it in the back of a garbage truck. The bed frame bounced off the truck and struck the left side of the employee's head, resulting in a concussion and a laceration.

An employee was working as an umpire near second base at a baseball game when they were struck in the head by a baseball thrown from the outfield. The employee sustained a subarachnoid hemorrhage, displaced temporal bone, and nondisplaced fracture of the left zygomatic bone.

An employee was being trained on a winding machine when his glove became stuck on tape. While freeing his glove, the machine was activated and started winding. His arm was pulled into the machine, resulting in a fractured left forearm.

The injured employee was cleaning up tools and equipment around a transformer that was being assembled. Other employees were removing the mobile work platforms from around the connections lattice of the transformer. The connections lattice fell over on top of the injured employee, causing fractures to their ribs, pelvis, and foot.

An employee was preparing to remove enamel from copper leads by soaking them in a liter container of formic acid when one of the bottles fell. The employee was splashed with formic acid, resulting in first- and second-degree burns to his face.

An employee was using a crane and an L-shaped lifting device to remove a coil of aluminum material off a de-reeler. The L-shaped lifting device got hung up on part of the de-reeler. The L-shaped lifting device slipped off and struck the employee in their right side. The employee sustained internal lacerations to their liver and kidney.

An employee was setting up a machine. The machine's jog function was activated while the employee's left hand was on the rotating table of the machine, and the hand was pulled into the next station of the machine. The employee was hospitalized with a laceration to the wrist that cut an artery, and they required surgery. The machine was not locked out/tagged out at the time.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
