An employee removed a rubber boot from a rail car and used his hand to push carbon back into the auger below the rail car. The tip of the employee's left index finger was caught in the auger.

An employee was moving a loaded lift from an aisle when it became wedged in the aisle and abruptly stopped, throwing the employee. The employee suffered a concussion, and fractures to two neck vertebrae and the left scapula.

An employee was in the basket of an aerial lift while unloading it from a flatbed trailer. The brake released, causing the aerial lift to strike another lift. The employee sustained a partially collapsed lung, rib fractures, and bleeding from the kidneys.

An employee was loading and unloading palletized material with a powered pallet jack. The pallet jack backed up and the employee's leg was crushed between the pallet jack and a bumper/trailer hitch, resulting in fractures to the right ankle and left tibia.

An employee was operating a forklift and went to grab a hold of the swinging arm to swing it into place. His hand was caught between the forklift and the swing arm, resulting in a middle finger amputation.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Kumho Tire Georgia, Inc.

An employee was adjusting a pallet on a conveyor when it came off the conveyor and struck his right ankle, breaking the tibia and fibula. The employee was hospitalized.

At 2:45 p.m., an employee was outside the plant during a break before his next shift. He was speaking with an associate when he suddenly fell from the third-level steps to the ground, striking his head on the concrete. The employee was hospitalized due to internal brain bleeding, a wound to the back of his head requiring staples, and heat-related dehydration.

GOODYEAR TIRE & RUBBER CO.

Two employees were replacing a motor on an overhead conveyor. The old motor fell to the floor, causing a piece of the concrete floor to fly up and strike the injured employee in the face, around the eye. The injured employee sustained a right orbital fracture, a zygomatic arch fracture, and a sphenoid fracture. The employee was hospitalized and required surgery.

Titan Tire Corporation of Freeport

An employee was changing the bladder on a hydraulic press. As he lowered the bladder, his left foot was pinned between its bottom ring and the bottom mold of the press. The second toe on the foot was amputated.

Howard Industries Inc.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
