An employee was monitoring parts of an elevator being moved off a transport truck when a piece of the elevator fell and struck the employee, resulting in a fractured right ankle and fractured ribs.
While repairing an elevator's counterweight, the mechanic isolated the counterweight by wedging a tool under it. The wedging tool dislodged, the counterweight moved, and the mechanic got his hand caught by the moving cable into the guard of the sheave.
