An employee was monitoring parts of an elevator being moved off a transport truck when a piece of the elevator fell and struck the employee, resulting in a fractured right ankle and fractured ribs.

An employee was holding the toe board of an elevator while installing it. The toe board slipped and contacted their pinky finger resulting in a laceration and fracture to the pinky finger.

The wedging tool dislodged, the counterweight moved, and the mechanic got his hand caught by the moving cable into the guard of the sheave. The incident de-gloved his hand and caused the skin to be ripped from the bone.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An employee was standing on the edge of the dumpster while pulling out cribbage. The employee fell to the ground, sustaining a fractured elbow and hip. The employee was hospitalized.

An employee was using a magnetic drill to drill a hole on an elevator door. His left glove became entangled with the drill bit, which partially amputated his left thumb below the last knuckle and removed several inches of a tendon. The employee's left middle finger was also severely lacerated. He was hospitalized and received stitches.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

United Refrigeration Inc

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

Parker-Hannifin Corporation

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
