TGI Fridays

An employee was using a large pot to carry hot oil from the fryers. His left foot got caught on a hose that was on the floor and the pot fell. When the pot hit the floor, the hot oil splashed upward onto the employee's face, hands, and legs causing second-degree burns.

An employee was walking from their work station when he stepped on a wooden board and rolled his ankle. He sustained a pelvic injury that required surgery.

Loc Performance Inc.

While on a scissor lift, an employee was cleaning an overhead fan. The employee tripped over the hose of a wet/dry vacuum and his right hand struck the running fan blade resulting in amputation of the middle fingertip. The fan was not guarded at the time of the incident.

At 3:23 p.m. on March 21, 2022, an employee was returning to her workstation when she tripped over a plastic band with her left foot, suffered a broken right femur as she tried to regain her balance, and collapsed to the floor. She was hospitalized.

An employee was climbing down a track ladder. The employee's left foot twisted on a step as their other foot reached the floor, and the employee's shin was broken.

An employee was using a 10-foot step ladder to install roller shades. The employee fell from the third rung from the top, landing on the ground and suffering back and head injuries. The employee was hospitalized.

An employee was cutting a cast iron pipe when the pipe fell onto his right hand, resulting in amputation of the thumb. The employee was hospitalized.

While on a scaffold, an employee was transporting heated sealant material in a bucket. The employee tripped and fell, causing the heated material to splash onto his gloved hands. The employee sustained second-degree burns to both hands and his right wrist.

Valcourt Exterior Building Services of Pensacola, LLC

Three employees were pulling tension cable (similar to rebar) through the walls of a condo using a motor system. Two employees were needed to hold the cable while the third employee drilled the mounting locations. When the third employee activated the drill, the cables twisted. They caught the right index finger of one of the other two employees, amputating his fingertip.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

Circle Graphics, Inc.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
