An employee was performing maintenance tasks on a roof in the rain. As the employee was descending the ladder, he slipped off a rung and fell, landing on both feet. The employee's right heel and left ankle were fractured.

Two electricians/journeyman mechanics were doing preventative maintenance on a high-density storage system, an aisle with two cranes. They were preparing to lubricate the chains on one of the cranes. One of them was on the lift station, seated above the crane on a cross-support bar to perform lubrication and test the vertical limit switch. He tripped the vertical limit switch to begin the work. The crane and the lift station began to move up, and his right leg was pinned and broken between the frame of the crane and the crossbar of the lift station.

An employee was riding a three-wheel trike through the plant when he struck a forklift. He sustained a fracture to the face at the cheekbone and temple. He also sustained small cuts on his shoulder and hand and was hospitalized overnight.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was operating a UV coater machine when it began to make a grinding noise. The employee was investigating the vibration of the machine when a gear caught their right index finger and amputated the tip.

A temporary employee was operating a seal machine to seal paper bundles. A jam occurred when the paper and plastic did not properly align to secure the seal. The employee went to clear the jam and the blade on the machine amputated their right middle finger.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.
