An employee was threading a machine when his left thumb came in contact with a cutting blade, resulting in a thumb tip amputation. The machine was guarded at the time.
Two employees were rolling out film with a lay-on roller. While the film was being pulled from the roller, one employee's right middle finger was caught between the roller and the safety guard. Three-eights of an inch of the employee's right middle finger were amputated.
An employee slipped while descending a ladder and fell, landing on the ladder and then on the floor about 5 feet below. The employee suffered a compound fracture of the left ankle, a left shoulder dislocation, and a broken nose.
