An employee was stepping down from a two-step ladder when the ladder slipped out from underneath them. The employee fell and landed on a sidekick trailer jack, resulting in four fractured ribs, including two ribs with multiple breaks, and a pulmonary contusion.

An employee was on a 10-foot step ladder clearing a plastic jam in an overhead conveyor system. The employee lost balance and fell approximately 8-9 feet to the floor, resulting in a compound fracture of the left leg.

An employee was performing a roll transfer on a turret winder. After indexing the turret, she was about to separate the bag at the perforation but noticed that the bag had not adhered to the new core. She tried to reach over the finished roll (which was on the rear spindle) to tuck the bag onto the new core (which was on the front spindle). Her hand was pulled into the front spindle, and her right forearm was broken. She was hospitalized.

An employee was reinstalling a 200-pound drive motor shaft in a mold machine, holding the shaft while aligning it and inserting it into the machine's shaft hole. The employee's left little finger was caught between a bearing on the shaft and the hole; the tip of the finger was amputated below the first knuckle.

An employee disengaged a shaft cylinder and in the process severely pinched her right index finger between the shaft and the shaft collar.

An employee was changing out the plastic core on a portable winder machine. After replacing the core, the employee turned on the machine and began to wind some material onto the new core. The employee's right hand then contacted the chuck of the winder, resulting in a right index finger fracture and partial amputation. The machine was not locked/tagged out at the time of the incident.

An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.

An employee was handling a small platform ladder with three steps and two wheels in the front. The employee was found on the floor underneath the ladder. She was hospitalized with multiple fractures to the sacrum.

An employee was on a ladder four feet above the ground to install insulation. The ladder began to slide and the employee jumped to the shop floor, breaking his left tibia and fibula at the ankle. He was hospitalized, requiring surgery.

On July 21, 2025, an employee was working from a 10-foot step ladder while installing a 3-by-5-foot glass window pane on the outside of a new home under construction. The window fell from the frame and the employee went to catch it when the ladder slipped. The employee fell 5 feet to the ground and landed on small pieces of concrete left over from the foundation pour. The employee was hospitalized with a dislocated elbow and fractured wrist that required surgery.

At about 11:00 a.m. on August 17, 2025, a Sealer Air Corp. employee suffered a broken right tibia while stepping down from a conveyor to the floor 19 inches below.

A maintenance employee was repairing the labeler on the line when it fell on their head, resulting in a broken jaw.

An employee was troubleshooting around the secondary draw rollers of a bag machine when their left index, middle, and ring fingers contacted the rollers. The employee sustained a partial fingertip amputation to one of the fingers. The machine was not guarded at the time.
