On September 23, 2024, an employee was filling a hot glue tank with glue pellets when he sustained second- and third-degree burns to four fingers on his right hand. The employee was not wearing personal protective equipment at the time.

An employee was rewebbing a machine when their right little finger was caught in a nip roller and amputated. The machine was not locked out at the time.

An employee reached into an ingoing nip point and suffered a fractured finger.

An employee was adjusting a film roller in a vacuum forming machine. The employee was pulled into the machine, and all four fingers and part of the thumb on the employee's left hand were amputated.

On June 10, 2018, an employee was clearing a jam in the polybag machine. The machine cycled and the heated pins used to punch holes into the plastic bags amputated the tip of the employee's left index finger.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was installing a hydraulic extension cylinder in a tow truck when their leg was caught between the cylinder and the truck frame. The employee was hospitalized with a broken tibia, which required surgery.

An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

At about 11:00 a.m. on August 17, 2025, a Sealer Air Corp. employee suffered a broken right tibia while stepping down from a conveyor to the floor 19 inches below.

An employee was troubleshooting around the secondary draw rollers of a bag machine when their left index, middle, and ring fingers contacted the rollers. The employee sustained a partial fingertip amputation to one of the fingers. The machine was not guarded at the time.

An employee was clearing a jam from the rewinder when their hand was pulled into a rotating part of the machine, fracturing one finger and dislocating another.

An employee was stepping down from a two-step ladder when the ladder slipped out from underneath them. The employee fell and landed on a sidekick trailer jack, resulting in four fractured ribs, including two ribs with multiple breaks, and a pulmonary contusion.

An employee was pushing a cart when they tripped over their feet and fell into the cart. The employee was hospitalized with injuries to their right hip.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
