An employee was rewebbing a plastic film printing press when his left index finger was caught in a chain and sprocket. The fingertip was amputated.

Two employees were rolling out film with a lay-on roller. While the film was being pulled from the roller, one employee's right middle finger was caught between the roller and the safety guard. Three-eights of an inch of the employee's right middle finger were amputated.

An employee was threading a machine when his left thumb came in contact with a cutting blade, resulting in a thumb tip amputation. The machine was guarded at the time.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

At about 11:00 a.m. on August 17, 2025, a Sealer Air Corp. employee suffered a broken right tibia while stepping down from a conveyor to the floor 19 inches below.

A maintenance employee was repairing the labeler on the line when it fell on their head, resulting in a broken jaw.

An employee was troubleshooting around the secondary draw rollers of a bag machine when their left index, middle, and ring fingers contacted the rollers. The employee sustained a partial fingertip amputation to one of the fingers. The machine was not guarded at the time.

The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
