An employee was servicing a pneumatic pump. After he attached an air hose, the air control valve caught his right middle finger, causing a partial tip amputation with no bone loss.

An employee fractured a rib when he slipped and fell on a hose reel.

An employee tried to reconnect an airline hose to a tilt table after using the hose to clean a piece of equipment. The table had been switched on, and it began to move to a vertical position while the employee was leaning under it to connect the hose. The employee was pinned between the table support beam and the front edge of the table. The employee suffered broken vertebrae in the back and was hospitalized.

On July 8, 2017, an employee was adjusting a part onto a CNC machine. He used a foot pedal to actuate an air clamp to hold the part in place. His left middle finger was caught between the part and the clamp, amputating his fingertip.

An employee was assisting in a saw re-build. The employee tried to grab a ball screw as it was falling and his finger was crushed/pinched between the falling part and the machinery housing.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was using a hydraulic bearing puller (held by a chain hoist) to remove a bearing from a railroad axle. The hydraulic bearing puller fell from the hoist (approximately 18 ) and struck the employees left foot, fracturing it.

An employee had been making deliveries. While unloading material from a tractor trailer, the employee sustained symptoms of heat-related illness. The employee was hospitalized.

An employee was loading/unloading product from a trailer at a customer location when he fell 10-12 feet from the top of the trailer to the concrete ground below. The employee sustained a fractured left ankle.

The employee was climbing into the driver side of the truck in the parking lot while holding a lunchbox and a clipboard. The employee slipped on snow and fell to the ground, fracturing a hip.

An employee was making a delivery and was pulling the customer's freight to the rear of the trailer using the plastic strap around the freight. The strap broke, causing the employee to fall from the rear of the trailer onto the asphalt pavement below. The employee sustained fractures to their left shoulder, elbow and pelvis.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
