An employee was using a manual pallet jack to move a catwalk frame to the overhead crane. The employee slipped and fell backward onto the ground, resulting in an L1 compression fracture.

InterCon Construction, Inc.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

RAVA Construction, LLC

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

Stein, LLC

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

7 Site & Utility, LLC

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

Sterling Steel Company, LLC

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

TRC Services of Texas INC

An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

Onpoint Industrial Services Hiring Center

An employee was using a powered miter saw to cut 2x4's. The saw blade hit something inside a board (possibly a hidden nail) and jumped, causing a laceration to the employee's left forearm. The employee was hospitalized with soft tissue and tendon injuries that required surgery.

A crane had been used to set a steel plate on a steel frame. An employee was trying to remove a clamp from the plate when the plate fell and crushed the tip of his left little finger. The employee sustained a fingertip amputation.

Morgan Engineering Systems, Inc

An employee was operating a scissor lift, raised to a height of about 15 feet, to inspect an overhead crane rail. The crane itself was being moved. Its hook swung into the scissor lift s arms, causing the lift to tip over onto its side, which in turn caused the lift and the employee to fall to the floor. The employee suffered injuries including broken ribs, spinal fractures, scapula fractures, and internal injuries.

Fairbanks Morse, LLC

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
