An employee was removing the inside steel pieces of a 91-inch by 91-inch by 6-foot concrete form, breaking the form down. The floor piece of the form (weighing more than 1,000 pounds) fell onto the employee's right side, lacerating his liver and injuring his right kidney and spleen.

An employee's finger was between two steel containers. When a forklift struck the steel container the employee's finger was crushed between the two containers and amputated at the hospital.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
An employee was anchored to a movable piece of equipment called a "lid" with a self-retracting lifeline harness. An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. The employee reached to steady it and the jacket moved downward pinching his left little finger between the jacket and the core and resulting in a fingertip amputation.
