Federal OSHA recorded a severe workplace injury at General Motors, 3201 Fairfax Trafficway, KANSAS CITY, KANSAS 66115 on — Amputations, affecting the fingertip(s).

Two employees were working on a tooling cross bar section. One employee had his right hand on top of a pneumatically controlled front tilt head. The tilt head actuated and pinched the tip of his right ring finger resulting in amputation of the fingertip.

An employee was moving a tool box when its top drawer started to open. While the employee was stabilizing the tool box and drawers, their left middle and ring fingertips were amputated through the nailbeds. The fingertips were surgically reattached.

An employee was assessing an overhead stamping press door for alignment issues when the counterbalance moved and their left thumb was caught between the chain and the sprocket. The thumb was amputated. The door was locked out/tagged out at the time but gravitational pull was not controlled.

An employee dropped off an empty dolly train and moved his tugger over to a full material train that was waiting. He was then walking around the material train to validate it, and was struck by a forward-moving forklift. The forklift tire rolled over the employee's right foot/toes, fracturing four toes. The employee was hospitalized.

An employee was conducting assembly line activities on the front driver's side of a van when she tripped and fell backward to the floor, landing on her back. The employee sustained a compression fracture to a vertebra.

An employee was driving a forklift when it struck a structural steel column, which separated the mast from the forklift. The right side of the employee's skull struck a front support post in the forklift cabin, and the employee suffered a neck fracture and a concussion.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
