An employee was anchored to a movable piece of equipment called a "lid" with a self-retracting lifeline harness. An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. His body and head struck the wall. The employee lost consciousness and sustained a laceration to the side of his head that required stitches. The employee also sustained multiple fractured ribs and fractured vertebrae.

An employee was standing on the mill, by the cross-seam area, preparing to open a bag of flux to pour into the hopper. The machine's holding clamps engaged and the clamp crushed the employee's left foot pushing the steel toe protection plate into their left toe.

An employee was breaking a flange free from a pipe that was approximately 90" in diameter. When the pipe flange broke free it smashed the employee's left foot against an immovable object.

An employee was operating a paint machine when the pistons became stuck, so he applied lubricant oil to make them come loose. He was cleaning near the piston by hand when the piston suddenly came loose and caught the his right index fingertip, amputating it.

An employee was grabbing the end ring of a pipe to move it when his right hand was caught between that ring and a second metal ring, partially amputating his right middle finger.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

While removing a lifting hood from a manhole core (steel cylinder), the cylinder rolled and the employee's left little finger was caught between the crane hook and the lifting eye. The employee's little fingertip was amputated just below the nail.

An employee was moving a 4x4 box jacket with an overhead crane. The employee hooked a chain to the jacket and put tension on it. It spun awkwardly, striking the employee and knocking him over. The employee fell on the same level and sustained a fractured rib and lacerations to his lower left abdomen and backside.

An employee was guiding a forklift operator who was placing a 48-inch form jacket (open cylinder) over a core (solid, stationary cylinder). The employee was positioned on the ground adjacent to the core. As the suspended jacket was positioned over the core, a gust of wind caused the jacket to move. The employee reached to steady it and the jacket moved downward pinching his left little finger between the jacket and the core and resulting in a fingertip amputation.

An employee was at the bell packer when the scraper of the bell packer struck his hand. The employee sustained broken fingers and degloving injuries involving their right ring and little fingers.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
