On August 5, 2019, an employee was operating a stand-up forklift when the forklift struck a horizontal PVC pipe that is suspended by chain and used as an area barrier. The end of the pipe shattered and pieces fell to the floor. The employee sustained lacerations to the left side of the face and bone fractures resulting in surgery.
