Final narrative
The injured employee was directing another employee (fork truck operator) on the placement of a 12' x 8' container onto a flatbed trailer. The employee was pointing to the place on the trailer where the container needed to be in order to secure it for transport. When the injured employee moved his hand, he felt a tug on his right index finger. He observed that the tip of his right index finger had been amputated.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
