An employee was performing a paper roll change at a paper sheeter machine. He was turning a non-driven unwind stand and the roll by hand when his left little fingertip got pinched between a machine depression in the chuck and the sensor. The employee's fingertip was partially amputated.

An employee was assisting the operator of a bag machine. The employee attempted to unjam material stuck in the machine when the machine started and the cut-off knife contacted the employee's left middle finger, resulting in a fingertip amputation with bone loss.

An employee was hand-feeding the end of a plastic roll into the nip point of a forming roller machine when the machine amputated the employee's left middle finger and partially amputated the left ring finger.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

An employee was walking when she tripped over a box on the ground and fell. The employee was hospitalized with a severe dislocation to the hip and leg that required surgery.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
