The arrow board was mounted on the TMA in the upward position with the arrow activated instructing the traffic to move over. The pilot vehicle with a trailer attached was approximately 102 feet in front of the TMA. The employee was between the pilot vehicle and the TMA, standing on the berm of the road with one foot on the berm and one foot on the fog line. A motorist went around the TMA and then took evasive action, came back across the lane, and struck the employee. The employee was hospitalized with broken ribs, a broken leg, and a head injury.

An employee was directing traffic when a shag truck turned sharply and ran over the employee. They suffered a crush injury to the lower left leg and internal bleeding.

The injured employee grabbed the grating to pull it toward himself and another employee was moving to the other side to assist with the lift. As the injured employee pulled, his right foot slipped and twisted. He was hospitalized with a fractured right leg.
