An employee was flagging traffic to slow down for a customer operating an excavator. The excavator reversed and ran over the employee's feet. The right foot sustained fractures and the left foot was injured.

An employee was setting up a lane closure. A Truck Mounted Attenuator (TMA) was stationed in the roadway with the crash cushion in the downward position. The arrow board was mounted on the TMA in the upward position with the arrow activated instructing the traffic to move over. The pilot vehicle with a trailer attached was approximately 102 feet in front of the TMA. The employee was between the pilot vehicle and the TMA, standing on the berm of the road with one foot on the berm and one foot on the fog line. A motorist went around the TMA and then took evasive action, came back across the lane, and struck the employee. The employee was hospitalized with broken ribs, a broken leg, and a head injury.

An employee was sitting on the bed of the work truck while picking up safety cones in a work zone. An SUV drove into the work zone and struck the work truck. The SUV then hit the employee's legs, knocking her to the ground. The employee was hospitalized with a leg injury.

On July 3, 2023, an employee was putting ice pallets away with a forklift when another forklift collided with the employee's forklift and crushed their left thumb, resulting in an amputation.

An employee was operating a riding lawn mower inside of a construction zone on the highway before new asphalt was laid. A vehicle entered the closed lane, striking the employee. The employee sustained blunt force trauma to the body, abrasions to the arms and back, lacerations to the head, and an open leg fracture.

An employee was walking and taking post-locate photos. He stepped on a ceramic drainage tile and his foot/leg punched through the tile. His leg was lacerated by the broken tile. The employee was hospitalized with internal bleeding due to a punctured femoral artery.

WG Yates Heavy Division*

Employees were preparing to place a section of metal grating measuring approximately 50" x 18" and weighing 40 to 50 lbs. The injured employee grabbed the grating to pull it toward himself and another employee was moving to the other side to assist with the lift. As the injured employee pulled, his right foot slipped and twisted. He was hospitalized with a fractured right leg.

An employee was in their personal vehicle awaiting instructions from the construction contractor on posting traffic cones. The driver of a motor vehicle swerved to avoid a steel plate in the roadway and struck the employee's car. The employee was hospitalized with four fractured ribs.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

Circle Graphics, Inc.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
