On August 21, 2019, a temporary employee was applying grated cheese to a pizza crust moving on a conveyor line to a plastic wrapping system. He was attempting to grab grated cheese that drops into a catch bin under the conveyor when his finger was caught in the chain and sprocket system, resulting in a finger amputation on his right hand.
Two employees were loading cardboard into a baler. The overhead door was closed, but the baler did not read that it was closed properly. When the door was re-closed, it caught an employee's right hand, causing an amputation to the index fingertip/print.
An employee was installing a safety block in the dough press. The employee was attempting to insert a safety block into the press when the press came down while the employee's fingertip was between the block and the frame. The employee suffered a right middle fingertip amputation.
