An employee was repositioning a crane to a different location when the crane tipped over onto its side. The employee jumped off the crane 10-12 feet to the ground below, resulting in multiple fractured back vertebrae.

On 9/9/2021, an employee was inspecting an array for defects when he was bitten on the leg by a snake.

An employee was aligning the butt section of a wind turbine when his left middle finger was struck by the reaction arm when it extended, resulting in amputation to the first knuckle.

An employee was using a torquing tool to torque down bolts on the blades in a wind turbine. As he began to tighten a nut, his finger became caught between the torquing tool and the wall of the hub. The employee sustained an avulsion fracture to a finger.

An employee was removing bolts from a crane to disconnect the crane from a wind tower. When the crane was being lifted up, the employee's left hand caught on a bolt and was pulled into the bolt, resulting in an amputated thumb and ring finger.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee was ascending the wind tower when their right arm was caught between the lift and the ladder. The employee sustained a right elbow injury and was hospitalized.

On 3/27/2025, at 9:57 AM, an employee was performing switching work including installing electrical grounds when an arc flash occurred. Personal protective equipment (PPE) was worn at the time, including an arc flash hood, shirt, and pants. The employee suffered burns and was hospitalized.

An employee was cleaning dirt out of a contactor using a hand-held blower and contact cleaner. An arc flash occurred, and the employee sustained burns to the right side of their face and both hands.

On February 16, 2025, an employee was descending a ladder on the third deck after completing rigging at a wind farm. Their left foot slipped on the penultimate rung and their leg became wedged between the ladder rungs and a horizontal bracket. The employee then landed on the second deck resting platform, resulting in a leg fracture above the ankle.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.
