An employee was standing on top of a feed truck attempting to loosen something with a pipe wrench. The pipe wrench slipped out of the employee's hand and he slipped and fell to the ground. The employee was hospitalized with fractures to an arm, hip, femur, and heel.

Employees were attaching a new liquid valve to an ammonia nurse tank. The wrench slipped and one employee fell off the side of a truck bed, landing on the ground and suffering a head laceration and spinal cord swelling.
