Federal OSHA recorded a severe workplace injury at General Dynamics, 423 Pacific Avenue, BREMERTON, WASHINGTON 98337 on — Amputations, affecting the fingertip(s).

Onboard an aircraft carrier, a rigger helper was steadying a pipe in a compartment while pipefitters adjusted the pipe in another compartment. While adjusting the pipe, the light end of the pipe shifted upwards and the rigger helper s left little and ring fingers were caught between the pipe flange and the overhead. The tip of the pinky finger was crushed and the fingernail was removed, and the ring finger sustained a 2-3 laceration. The employee was hospitalized and the pinky finger was amputated to the base of the fingernail.

A rigging hook had just been unhooked from a pipe that a crane had lowered. While the crane's cables were being raised, the hook caught onto the racking that was bracing the pipes. The tip of the hook broke off and struck an employee in the jaw, lacerating and possibly breaking it.

Employees were unloading logs on a logging vessel. The injured employee was standing on the deck as a load came in. The sling popped off the load and the end of the choker struck the employee's lower left leg, resulting in a shin fracture.

An employee was exiting a confined space while carrying a pressure gun when the trigger activated. Water penetrated the employee's left arm, resulting in injury.

The employee was putting shackles on the grab of a vessel-operated crane. As the crane was lifting the grab, it started to swing toward the hopper. The employee reached to stop it when they were caught and lifted by the grab and pinned between it and the hopper. The employee sustained a pelvic fracture and lacerations to the arm and pelvis.

An employee was launching a boat during rough sea conditions. The employee lost balance, causing their left index and middle fingertips to be caught in the eye of the bow line, resulting in amputation.

Two employees were in a boat pulling a 45-pound anchor out of the water by the rope. The rope tightened and as the employees were attempting to create slack in the rope, the injured employee began to slip. His hand was caught between the rope and the inside of the platform boat, resulting in amputation of the right middle finger with bone loss.

The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

An employee was drilling a hole into the sealed hold of a vessel so a marine chemist could test its atmosphere. Upon drilling the hole, a gas that was under pressure was released and caught fire at the drill motor, where the employee was holding it. The employee was burned.

Navy NAVFAC-NW

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

An employee was testing and troubleshooting the gang cylinder valve located in the gang area of the sawmill. The employee's fingertip was amputated by the gang cylinder valve.

An employee was unlocking containers on deck. The employee was de-lashing while standing on the edge of the lid. The lashing bar he was holding came off the cone, causing him to turn 90 degrees and strike a railing behind him. The employee sustained four fractured ribs.

JONES STEVEDORING COMPANY

An employee parked a side-by-side on the fixed internal ramp aboard a cargo ship. They exited the side-by-side and went to inspect the cargo below. The side-by-side rolled toward the employee and struck them as they were walking toward the edge of the ramp. The employee was knocked off the ramp and landed on the lower deck approximately 8 feet below. The employee was hospitalized with a fractured shoulder.
