An employee was riding a power trowel to move it out of the way when the power trowel got caught in safety netting. The employee and the power trowel fell 18 feet off the second deck and the employee sustained a ligament injury to the right foot, a laceration/bruise on their liver, and a foot injury. The employee was hospitalized.

An employee was pinned between two concrete molds while checking on a kiln issue, resulting in severe leg injuries.

An employee's hand/arm was entangled in a conveyor belt. The employee was hospitalized.

An employee was setting forms for a concrete pour. A steel plate that shortens the form was askew in the track and did not seat properly. The employee began to wiggle the plate to get it in place when it released and pinched his left ring finger, resulting in a fingertip amputation.

An employee was attempting to unclog/unjam a spray paint gun. When he pulled the trigger to verify if it was unclogged, his left middle finger was injected with paint.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was sitting on the tailgate of a moving pickup truck while holding the handles of a wheelbarrow. The employee fell off the tailgate and struck his head on the asphalt. The employee was hospitalized with a fractured skull and a brain bleed.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
