An employee was operating a stand-up powered industrial truck when their left foot was crushed between the forklift and a support beam, resulting in fractures to multiple bones in their foot.

An employee was removing cardboard that was caught in the top of a cage while standing on the outside of the powered industrial truck (PIT). The employee's left hand was crushed between the PIT fork and bar.

An employee was pulling a bulk-bag of peanuts weighing approximately 1,700 pounds using a forklift when another bulk-bag destabilized and fell on the cage of the forklift. The employee jumped from the forklift and when the bulk-bag broke open it rolled and landed on him, pushing his face to the floor. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.
