Final narrative
An employee depressed the foot pedal on an air bending machine to bend parts. He rested his left hand on the area where the arm meets the bending roller, which controls the bending arm. When he released the foot pedal, the bending arm automatically returned to home position and his left thumb was pinched between the arm and the bending roller. His left thumb tip was amputated as a result.
An employee was working on a resistance welder. The welder activated, and a copper pin on it caused an amputation to the employee's left middle finger.
A technician was troubleshooting a multi-spindle CNC lathe which is used for metalworking. The technician was manually spinning the spindle which causes the belt and pully to spin. The injured employee s hand became caught in the machine resulting in an amputation to the left index finger.
