An employee was operating a machine and went to remove a buildup of desiccant from the machine. Their right index finger was pinched between two moving parts, resulting in an amputation to the finger.

An employee was manually removing a bearing from a vertical washer brush. Due to the weight of the brush roll, it released and crushed his left ring fingertip resulting in amputation without bone loss.

As he pulled on it, the belt started to turn faster than the employee expected and his left little finger was caught between the high-tension belt and the pulley on the motor. The employee was checking an airlock to determine whether there was suction when their left index and middle fingers were pinched between the rotating rotor and the housing of the airlock. The employee suffered amputations of both fingertips without bone loss.
