Final narrative
An employee was removing bolts on a shaft adapter with a ratcheting allen head socket wrench. While the employee was loosening a bolt, the blocking bar for the shaft slipped, allowing the shaft to drop 6 inches and pinch the employee's left little fingertip between the ratchet and the top of the gearbox. The finger was partially amputated.
The injured employee was talking to another employee while kneeling on the mud pan. The injured employee's right foot/leg were caught between the mud pan and the catwalk resulting in a fracture to the right leg. The machine was in operation at the time.
