An employee was conducting wildlife surveys from a UTV. After attempting to dig out the UTV, she was climbing back into the vehicle and had her hand on the roll bar when her foot slipped off the step-up plate, causing a sudden sharp pain to her abdomen area. She was in severe pain and required hospitalization for a muscle injury.

An employee was stepping on an aluminum cross member to exit a boat in a building when his foot slipped off, causing his shin to strike the cross member bracket. The employee sustained a shin laceration and fracture.
An employee was dismounting an excavator after loading it onto a lowboy trailer. The employee slipped, but managed to catch the grab handle to avoid falling. The sudden stop ruptured a cyst in the employee's abdomen.
An employee was adjusting the high/low switch on his truck-mounted welder. He was turning around when he lost his footing because it had just rained and his truck bed was slick. He went to steady himself and reached to grab onto something with his hands. His right hand slid underneath the cowling of his welder and two fingers were severely lacerated by the motor fan blades. He was hospitalized.
The employee was working in the back of a micro truck and maneuvering around the materials in the back of the truck. The employee's foot caught on the edge of the truck, causing him to lose balance and fall from the back of the truck to the ground approximately 3.5 to 4 feet below. The employee landed on his feet on the loading dock but rolled his ankle. The employee was hospitalized with a compound fracture of the ankle.
An employee was assisting the operator of a soil sampling rig to collect soil samples. He went to remove soil sampling tooling and the energized equipment cycled. His right index fingertip was amputated including the fingernail.

An employee was sampling a fluid catalytic cracking (FCC) unit by opening a valve and placing a proving tube inside to collect the sample. The sample taken is a hot gas mixture. When the sample was collected, the flange came undone and fell due to the pressure. The employee sustained an amputation to their right middle fingertip.
