An employee was standing on the floor inspecting parts for paint quality. When he was finished looking at a part, he starting walking backwards and tripped on a wooden pallet that was laying on the floor behind him. He fell to the concrete floor and fractured a hip.

An employee was winching in the mid-section of a boom on an aerial lift into the base section when their left middle and ring fingers were pinched between the two sections of the boom subassembly. The employee sustained a displaced fracture of the middle phalanx of their left middle finger and the amputation of their left ring fingertip.

An employee was preparing to use a programmable automated vertical band saw. The employee was clamping a piece of angle iron in place on the machine when the clamp caught the employee's left index fingertip. The employee suffered an amputation to the fingertip, up to the edge of the nail.
