An employee was winching in the mid-section of a boom on an aerial lift into the base section when their left middle and ring fingers were pinched between the two sections of the boom subassembly. The employee sustained a displaced fracture of the middle phalanx of their left middle finger and the amputation of their left ring fingertip.

An employee was preparing to use a programmable automated vertical band saw. The employee was clamping a piece of angle iron in place on the machine when the clamp caught the employee's left index fingertip. The employee suffered an amputation to the fingertip, up to the edge of the nail.

An employee was standing on the floor inspecting parts for paint quality. When he was finished looking at a part, he starting walking backwards and tripped on a wooden pallet that was laying on the floor behind him. He fell to the concrete floor and fractured a hip.

A welder was removing a steel production part from a welding fixture using an overhead crane. He connected the lifting hook to the steel part's center post, then applied tension to the overhead crane. His little finger was pinched between the lifting hook and the steel part, causing the amputation of the tip.

An employee was assembling a boom lift, inserting and sliding a section of boom into another boom section. The underside of her left middle finger was pinched the two sections, resulting in a partial amputation to the fingertip.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

Employees were building a gantry. The injured employee was carrying part of the gantry when their left ring finger was crushed between the gantry and wood. The employee's fingertip was amputated.

A crane operator slipped and fell while getting out of his crane, landing on the ground and rendering him unconscious. The employee was hospitalized.

A civilian vehicle entered the construction site to avoid a motor vehicle accident and struck two employees who were cleaning up the area. The two employees sustained injuries and were hospitalized.

An employee was assisting the drill rig operator when his left little finger was crushed by the Kelly bar, resulting in amputation of the finger.

An employee was removing a blade that was lodged in a pipe when their foot was caught by the cold cutter blade attachment. The employee sustained a crush/laceration injury to three toes on the right foot. The employee was hospitalized.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
