Final narrative
On July 6, 2020, a mobile equipment operator was operating an overhead crane inside a building and felt light-headed. The employee then took a 15-minute break and drank a bottle of water. After the break the employee was standing and fell to the ground breaking his left ankle. The employee was hospitalized for heat exhaustion.
An employee was using an overhead crane to lower a manhole cylinder to the ground to complete maintenance and cleaning. While removing a lifting hood from a manhole core (steel cylinder), the cylinder rolled and the employee's left little finger was caught between the crane hook and the lifting eye. The employee's little fingertip was amputated just below the nail.
On April 26, 2023, an employee was using a jack to raise a machine carousel to replace a bushing. The jack slipped, causing his right little finger to become pinched between the jack and the carousel. His right little fingertip was amputated without bone loss.
An employee was removing cured product from casts and staging it in the yard for future delivery when one of the outer casts opened up crooked. The employee placed his left hand on scaffolding for the outer cast. When the employee attempted to correct the crooked cast by pushing it, it came back together and trapped his middle finger. The employee sustained injuries to the skin and muscle.

The employee reached to steady it and the jacket moved downward pinching his left little finger between the jacket and the core and resulting in a fingertip amputation.
