On July 6, 2020, a mobile equipment operator was operating an overhead crane inside a building and felt light-headed. The employee then took a 15-minute break and drank a bottle of water. After the break the employee was standing and fell to the ground breaking his left ankle. The employee was hospitalized for heat exhaustion.

While removing a lifting hood from a manhole core (steel cylinder), the cylinder rolled and the employee's left little finger was caught between the crane hook and the lifting eye. The employee reached to steady it and the jacket moved downward pinching his left little finger between the jacket and the core and resulting in a fingertip amputation.
