An employee was stepping down from a two-step ladder when the ladder slipped out from underneath them. The employee fell and landed on a sidekick trailer jack, resulting in four fractured ribs, including two ribs with multiple breaks, and a pulmonary contusion.

An employee was on a 10-foot step ladder clearing a plastic jam in an overhead conveyor system. The employee lost balance and fell approximately 8-9 feet to the floor, resulting in a compound fracture of the left leg.

An employee was performing a roll transfer on a turret winder. After indexing the turret, she was about to separate the bag at the perforation but noticed that the bag had not adhered to the new core. She tried to reach over the finished roll (which was on the rear spindle) to tuck the bag onto the new core (which was on the front spindle). Her hand was pulled into the front spindle, and her right forearm was broken. She was hospitalized.

An employee was reinstalling a 200-pound drive motor shaft in a mold machine, holding the shaft while aligning it and inserting it into the machine's shaft hole. The employee's left little finger was caught between a bearing on the shaft and the hole; the tip of the finger was amputated below the first knuckle.

An employee was changing out the plastic core on a portable winder machine. After replacing the core, the employee turned on the machine and began to wind some material onto the new core. The employee's right hand then contacted the chuck of the winder, resulting in a right index finger fracture and partial amputation. The machine was not locked/tagged out at the time of the incident.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

A subcontracted engineer was advising maintenance how to make a modification to the embossing roll system on a new line. Maintenance was bringing the motor and gearbox down with a crane. The load shifted when it was a few inches from the ground. The engineer went to catch/maneuver the gearbox and the fingers on his left hand were crushed between the I-beam base of the gearbox and the concrete floor. A finger was amputated.

On July 30, 2025, an employee was assisting co-workers in using a hoist to position an injection molding tool onto a work table. As the tool was being lowered, the slide section shifted, trapping and amputating the employee's right thumb tip.

An employee was manually lifting a film roll when their right middle fingertip was pinched between the roll and a metal beam, resulting in a fingertip amputation.

3M Company

At about 10:00 a.m. on July 13, 2025, an employee was cleaning the adhesive from running rollers using a rag soaked in cleaning solvent to prevent contamination. The rag was caught between two rollers and pulled the employee's right hand between the rollers. The employee sustained right hand crushing injuries, de-gloving injuries (palmer and dorsal sides), and fractures. The employee was hospitalized and required surgery. The machine was not locked out/tagged out at the time.

Simona America Inc.

An employee working on a sheet line was removing masking from the underside of plastic sheeting that was being lifted by a vacuum lift. The plastic sheet measured 4 feet wide, 14 feet long, and approximately 1 inch thick, and it weighed approximately 300 pounds. The vacuum lost suction and dropped the plastic sheet onto the employee's arm, pinning it between two sheets and resulting in a broken right forearm.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
