An employee was performing maintenance on the refrigeration unit of a truck. As they were walking up a platform ladder, the ladder fell backward and the employee reached up with his right hand to hold onto something. The employee contacted the refrigeration motor belt, which pulled his index and middle fingers into a pulley, resulting in amputations to the first knuckle.

An employee was unhooking an air hose from an oil pump in a shop. As he disconnected the quick coupler, the compressed air caused an abrasion to his right ring finger.

An employee was performing a maintenance check when the reefer engine turned back on as the employee was checking the belt tension. The employee suffered open fractures to their hand and one finger.

An employee tripped and fell in a concrete parking lot for rental vehicles, resulting in a broken femur.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee had dumped a load of hot mix (tar) and was cleaning the interior walls of a belly dump trailer onsite. The employee was scraping the walls of the trailer when the doors articulated, breaking the securement pin and pinching the employee's legs. An employee was transferring sulfuric acid into a customer's tank. They connected the transfer hose to the receiving tank and attached the compressed air line used to pressurize the trailer. An air valve opened during unloading and a small amount of sulfuric acid got under the employee's face shield and onto their face. The employee suffered third-degree chemical burns to their face, right ear, and chest.

An employee was working to adjust a misfed pallet board that was entering the pallet board collection point. His left middle fingertip became caught between the board he was adjusting and other boards already stacked in the collection area. This resulted in amputation of the fingertip.

On July 22, 2025, at approximately 4:30 a.m. after the previous day's shift, an employee was hospitalized with a kidney injury caused by heat-related illness. The employee had worked several consecutive days delivering packages in high outdoor heat and humidity without cooling gear.

On July 18, 2025, an employee was tarping a load on a flatbed trailer in the yard. He fell approximately 13 feet off the loaded trailer and landed face-down on a gravel lot. The employee sustained facial cuts, abrasions, scrapes, fractures to their face, as well as several fractured ribs. He was hospitalized.

An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee tripped on a rotor on the floor of a store, fell, and suffered a broken right thumb.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.
