Employees were looking at an intake fan problem on a press. One employee was testing the tension on a belt when his right index finger was caught between the belt and the pulley. He suffered a partial amputation to the finger.

An employee was removing a jammed bundle from a conveyor when their finger was pulled by a sheet metal guide. The employee sustained an amputation of the right ring fingertip.

An LSC Communications security guard was performing security checks throughout the facility while operating a three-wheeled electric cart. The cart struck a vertical steel I-beam, causing the employee to continue forward and contact the edge of the I-beam and then fall to the concrete floor. The employee was hospitalized with lacerations on the forehead and nose, and fractures to the forehead and right cheek bone.

An employee was placing a belt back on the track when the machine cycled and caught the employee's right index finger between the side of the pulley and the stationary table top resulting in amputation above the first knuckle. The machine was not locked out/tagged out at the time.

An employee was spraying concrete sealer when they fell into a 3-foot drain hole. The employee landed on their feet and sustained a broken femur.

A temporary employee walked across a wooden pallet to retrieve product to stock. When he walked back across the pallet, it broke, causing the employee to fall to the ground and sustain a fractured hip.

On April 10, 2023, at 12:00 PM, an employee was using a broom to clear rock from a building's foundation in preparation for a spray-on vapor barrier. There was a 4-foot deep trench next to the building where the employee was working and the soil at the trench's edge fell away. The employee fell into the trench and was hospitalized for a left pelvis and/or femur fracture.

An employee was walking up a steel staircase to check wiring in the electrical room. The staircase had a top platform with a grate that was sitting across the beams of the platform. The grate had shifted and as the employee stepped on it, it gave way, causing him to fall approximately 3 feet to the ground below. The grate landed on the employee and he was hospitalized for four transverse process fractures in the lower spine.

An employee was stocking the freezer and stepped on a wood pallet. The employee's foot went into the pallet and they sustained a right ankle fracture that required hospitalization and surgery.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

The injured employee's standup forklift had mechanical issue with a tire. The injured employee was inside their forklift to help steer it while it was being pushed up the ramp by another forklift. As the other forklift started to move up the ramp, it's forks moved and went into the driver's space of the disabled forklift. The fork penetrated the injured employee's left shoe, fracturing the two middle toes and injuring the first toe. The employee was hospitalized and required surgery.

A press was having a paster failure and the crew was working to get the ink on the paper. A press operator grounded himself and was entering the press unit when something flashed. The employee sustained third-degree burns to his head and left arm.

Around 12:45 PM, a clamp truck operator went to remove a piece of orange strap from a clamp truck that was parked outside of the office. The strap fell into a gap between the frame and the hood of the truck. The strap became caught in the engine's fan and also wrapped around the employee's hand when he went to grab it. The strap was pulled tight against the frame of the clamp truck, causing lacerations and fractures to the employee's hand. The employee was hospitalized and required surgery.

At 12:00 p.m. on August 13, 2025, an employee was unloading a 1,100-pound roll of paper from a skid when the roll fell onto his right leg. The leg was broken.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
