Final narrative
An employee was taking down warehouse racking when a piece of the racking fell down and amputated the tip of the employee's index finger.
An employee was inside a metal container from which temporary panels (sitting on pallets in the container) were being pulled. The employee was holding on to the side of the panels to stabilize them as they were lifted by a forklift. The panels started to tilt and then fell onto his left leg. He suffered a broken femur and was hospitalized, requiring surgery.
An employee was fixing a jam in the wire extruding machine and caught the tip of their finger between the drum and roller on the wire roll up. The employee sustained a right ring fingertip amputation. The machine was not locked out/tagged out at the time.
Employees were removing a full stand of wire spooling and replacing it with an empty stand. As the injured employee was installing the retaining cap on top of the empty spool, it fell and amputated his right ring fingertip.
