An employee was fixing a jam in the wire extruding machine and caught the tip of their finger between the drum and roller on the wire roll up. The employee sustained a right ring fingertip amputation. The machine was not locked out/tagged out at the time.

The employee was holding on to the side of the panels to stabilize them as they were lifted by a forklift. The panels started to tilt and then fell onto his left leg.
