An employee was applying lubricant to a mold machine. The employee slipped and his hand contacted the machine resulting in the amputation of the left middle fingertip.

An employee was making a repair to a forklift. The tines of the forklift were elevated in order to access the brackets at the bottom of the backrest. While the employee was loosening one of the brackets, the backrest and tines fell off the forklift, striking the employee's back, legs, and feet. The employee was hospitalized with a fractured left leg that was broken in two places and a laceration to the right foot.

When the block came out, the frame came down on his right ring and little fingers. The employee sustained a fracture and partial amputation to the ring fingertip.

An employee was using a fork truck to move a tub containing molten iron that had been drained from the cupula. The tub tipped over and the molten metal came in contact with water. This caused the molten iron to splash back and burn the employee's upper torso.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.
