Final narrative
On 11/09/2020, at 11:45AM, an employee began to climb an extension ladder. Upon reaching the fourth rung he began to descend the ladder. His left foot slipped between the second and third rungs, causing the ladder to destabilize and fall. The ladder slid down the wall and the employee rode it to the ground, with his foot trapped in the ladder. He sustained a fractured left ankle.

While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
