On January 9, 2021, an employee was buffing a floor with an orbital buffer. The cord wrapped around his ankle and he fell backward, fracturing his skull.
An employee was checking for an air leak on an injection molding machine. The machine actuated, causing amputation to the employee's left index finger.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit.
