An employee was operating an excavator on the side wall of a trench when the side wall collapsed. The excavator fell approximately 8 feet into the trench and landed on top of a fuel tank. The employee sustained fractures to the clavicle, wrist, and ribs, as well as left wrist lacerations.
An employee was performing excavation activities when he was struck by the counterweight of an excavator, propelling him into the excavation. The employee landed on the boom of another machine, resulting in a fracture to his left hip that required hospitalization.
An employee was installing an under-dispenser containment sump when their left middle finger got caught between the sump and a bracket, resulting in a fingertip amputation.

It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.
