Final narrative
While walking towards the office inside the warehouse, an employee slipped on a piece of debris (green strap) that was on the floor and fell. The employee sustained a fractured right hip.
An employee was trying to remove a blockage in a dust collection system. The airlock fan activated and amputated three fingertips on the employee's right hand.
An employee was disconnecting a truck from a trailer. While pulling the fifth-wheel release that the trailer was attached to, the employee lost balance and fell backward onto the concrete pavement, resulting in a fractured T12 vertebra.
Maintenance employees were replacing a gear box in the stacker of a press. After the replacement was complete, the stacker was restarted. The maintenance employees then heard a loud noise coming from the drive chain area and the injured employee went to check the chain tension when the machine amputated his right index fingertip.
An employee was helping to change out a cylinder on a press. He was putting the cylinder arms down to put the cylinder in place. As he stepped over the driveshaft to pull the pin and roll the cylinders, he lost balance and started to fall forward.
