An employee was cutting bolts from a bridge during a bridge demolition. When the bolts were cut, the employee was pinned between a beam and the aerial lift. When the lift was moved to unpin the employee, they fell to the ground below. The employee suffered a broken leg and pelvis.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
