An employee was using a CNC lathe to finish-grind a small urethane roll. His right hand contacted the rotating grinding wheel and was pulled in. He suffered amputations to the ring fingertip (at the first knuckle) and the middle finger (at the second knuckle).

Finzer Roller, Inc.

An employee was pushing a cart holding 10 steel cores vertically. The cores were 44" high, 1" in diameter with a 1/4" keyway, and they weighed 10 pounds each. He stopped the cart which caused three of the cores to fall. The keyway of one the shafts fell onto his right middle finger that was resting on the cart and it amputated the fingertip.

Finzer Roller, Inc.

An employee was removing a roller weighing approximately 1000 pounds from the laser inspection lathe using a spreader bar with two wire-rope slings. The employee was using the lifting device and attaching the cable strap sling to the two journals of the roller so that she could use the hoist to move the roller to a box. When she raised the hoist to make the sling taut, her right index finger was caught between the sling and the roller. The employee suffered an amputation of her index finger to the distal knuckle.

An employee was operating a bar spinner machine when he noticed that it was not moving back and forth smoothly. The employee went under the machine while troubleshooting to look at how it was traveling along the rails. As the employee went to stand up, his hand came in contact with the machine while the spindle was still in motion, resulting in an amputation of the right index finger above the first knuckle.

An employee was grinding a rubber roller on a manual lathe. The employee's right hand came into contact with the grinding wheel and the employee's right middle and index fingers were partially amputated.

Rubber Right Rollers

An employee was operating a rubber roller grinder and had just completed the grinding process. He changed the rotation of the spindle to the clockwise position and began to clean rubber dust off the rollers with a shop rag. The rag caught on the roller and pulled his hand into the grinding wheel, resulting in a broken wrist, broken fingers, and lacerations to the hand and fingers. He was hospitalized.

An employee was clearing jammed material from an extruder machine. The employee reached their left hand through the unguarded opening between the extruder machine and the conveyor belt to manually dislodge the jam and contacted the moving chain and sprockets, resulting in an amputation to the left thumb.

At about 10:05 a.m. on March 10, 2025, an employee was working at the loading side of a mix conveyor when his right wrist was caught in a pinch point between the conveyor and a roller. He suffered laceration and fracture injuries to the wrist and was hospitalized.

An employee was manually adjusting a mold on a shuttle plate when the mold shifted, resulting in amputation of his right middle fingertip.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
