An employee was on a platform on a concrete truck cleaning dried ready-mix from the chute. The employee fell from the platform to the ground resulting in hospitalization for a fractured hip.
An employee was adding an anti-slip foaming agent to a paving transfer machine when the container of foaming agent fell into the machine's hopper. When the employee went to remove the container, his right arm was caught in the machine's rotating blades, resulting in amputation of the arm.
An employee was spraying belt dressing on a conveyor's head pulley when his glove got pulled into a pinch point. The employee sustained fractures to their facial area and lower right arm.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
