Final narrative
An employee climbed a ladder to work on the boom of a crane. The employee was striking the boom with a pry bar when he fell off the ladder and struck the ladder on the way down, resulting in a skull fracture, ruptured ear drum, multiple rib fractures, and other scrapes and bruises.
An employee was offloading truck-mounted forklift carriages to palletize and band them. He had moved one carriage and was putting another one on top of it when the second carriage slipped. His left hand was crushed between the two carriages, and he suffered cuts to the index and middle fingers. He was hospitalized, requiring stitches and tendon surgery.
